Forms that need to be completed every school year:
- Demographic Confirmation Form
- Yearly Consent Forms
- Technology Use
- Field Trip Consent
- Locker Use Agreement
- Medical Form - only submit if your child has a medical condition or requires medical treatment. Provide office with the medication/epi-pen etc.
